Your Family Health Matters — We’re Excited to See You at the Clinic!
Please ensure you complete the following items to ensure your visit is as smooth as possible.
- If you have an appointment for an annual physical or plan to get blood work done, come fasting to your appointment, but drink plenty of water.
- Bring your government issued photo ID
- Bring your health insurance card
- If you have not been to the clinic or used TextCare since February 12th, 2025, fill out this quick paperwork so your provider can prepare for your appointment
- If you’ve been receiving care from the Blue of Tennessee Center at Cameron Hill, you should obtain your medical records to take to your new provider(s) at the BlueCross Employee & Family Clinic. Complete the two forms below electronically and email the completed forms to twilliams1@mysanitas.com and jlundy@mysanitas.com.
