The BlueCross Employee and Family Clinic offers a wide range of services to support the health of BlueCross employees and their dependents. 

Medical services:

• $10 per visit for Group 44 employees and dependents on the PPO or HDHP plan
  – For PPO members: applied to your out-of-pocket maximum
  – For HDHP members: applied to your deductible and out-of-pocket maximum
• $45 per visit for non-Group 44 employees and dependents
   
• Note: The following services are free of charge for Group 44 employees & dependents:
  One annual physical or well woman exam per year, chronic condition management visits, labs, flu shots, allergy shots, T-Dap vaccines, steroid injections, Toradol, Zofran, vitamin B12, Rocephin, and TB skin tests.
• Note: The following services are free of charge for non-Group 44 employees & dependents:
  One annual physical or well woman exam per year and chronic condition management visits.

Physical therapy:

• $20 per visit for employees and dependents on the PPO or HDHP plan
  – PPO: applies to out-of-pocket maximum
  – HDHP: applies to deductible and out-of-pocket maximum
• $45 per visit for employees and dependents not on a BlueCross plan
   
• Note: Subject to the 60-visit limit per benefit year under all plans
